PrEP is an abbreviation for `Pre-Exposure Prophylaxis’

♦ PrEP is HIV infection prevention measure that reduces the risk of infection by taking anti-HIV drugs orally before the sexual intercourse. It is recommended to people who are at high risk of getting HIV.

♦ WHO says “Use of a condom is a preventive measure but may depend on your partner. PrEP is a way to protect yourself at your own will” What medicine is prescribed?

♦ The drug widely used as PrEP is “Truvada TDF/FTC”, which was approved by the US FDA in 2012. In October 2019, the drug “Descovy TAF/FTC” was approved.

There are two ways to take (medicine type taken are the same)

1. Daily PrEP = Taking medicine daily regardless of sexual activities Directions: 1 tablet once daily

2. On-demand PrEP = Medicine taken before and after sexual intercourse (Also called Event-Driven PrEP / Event-Based PrEP)

How to take On-demand PrEP:

・Example ①. If you have sex for only one day:

⇒Take two tablets 2 to 24 hours before sexual intercourse, then take one tablet 24 hours later, and another one tablet 24 hours later. Use 4 tablets in total for one sexual intercourse

・Example ②. If the sexual intercourse lasted several days:

⇒Take two tablets 2 to 24 hours before sexual intercourse, and 1 tablet every 24 hours thereafter, and continue for 2 days after the last sexual intercourse.

*The feature of On-Demand PrEP is to take 2 tablets first. This is done to make PrEP effective abruptly.

Preventive effect

♦ It is believed that if you take your medicine as adviced, it will have a preventive effect close to 100%.

However, as PrEP only has a protective effect against HIV, keep in mind the use of condoms and safe sexual behavior to prevent other sexually transmitted diseases.

Side effects of PrEP

・Originally, there were some concerns about renal dysfunction and bone mineral loss due to the side effects of long-term use of “Truvada”, but this new drug “Descovy” has almost no kidney concern.

・We tend to test renal functions everytime you visit us, only to see if there is any problem with renal function. 

・To be on the safe side, it is necessary to check whether you are taking medicines that may damage your kidneys. If you have any medicines that you are currently taking, please consult with the doctor.

If you want to stop taking your medication

・Please contact us in advance when you stop PrEP.

・In order to safely discontinue you should stop taking PrEP 2 days after your last risk act.

・Also, PrEP is a combination drug that contains a drug called tenofovir, which is also a therapeutic drug for chronic hepatitis B.

・In chronic hepatitis B, if one discontinues oral use of PrEP, hepatitis B may worsen rapidly. Please consult your doctor in advance if you stop taking PrEP.

At your First visit:

・You will discuss with the doctor what kind of PrEP plan is desirable.

・We will carry out HIV test, hepatitis B test, and syphilis test.

・1 bottle of PrEP (30 tablets) will be prescribed.

・Gonorrhea/chlamydia (urine, throat, and anus) or discharge examination (candida, trichomonas, and bacterial vaginosis) is available for an additional charge of 3,000 yen.

・For follow-up, we will repeat the above tests, and we shall prescribe 1 bottle, or 3 bottles PrEP set.

・Other tests: Hepatitis A, B and C antibody tests are available, as well as vaccinations for Hepatitis A & B, and also HPV 9 (gardasil 9) vaccine.

・PrEP 3 bottles (90 tablets)

・+ 3,000 yen on Discharge test (candida, trichomonas, and bacterial vaginosis)

3. PrEP 6 months set :  43,000 yen (incl tax)

・PrEP 6 bottles (180 tablets)

・On the spot test: HIV,  hepatitis B

・Syphilis , Kidney function test

・follow-up tests in 3 months done for free: HIV, syphilis, hepatitis B, Kidney function
